The Living Heart Foundation (LHF)
and many current professional football team doctors and
athletic trainers encourage you to participate in our
Cardiovascular (CV) Health Screening Program for active
and retired professional football players. There is no
cost to you for the health screening program. We are
privileged to be sponsored by the Professional Athletes
Fund and several distinguished national and regional
corporate partners. If you were to undergo our testing and
on-site health education programs in either a local
hospital or in your doctor's office, the average charge
for these services would exceed $4,000.00.
Mark your calendar for
this important upcoming event! The LHF and the NFLPA are teaming
up once again to encourage the NFL retired players to take the
initiative to help improve their health and increase the quality
of their lives.

Our voluntary health screening is
important to you because it will help you understand
your risk for developing cardiovascular and lung
problems and how you can limit the chances of serious
related complications and premature death in the future.
The mortality toll from CV diseases in the USA is
equivalent to a daily jumbo jet crash each day of the
year. The LHF plans to further help you during the
screening event by providing you with health education
and interactive sessions where your health questions can
be addressed.
If you decide to
participate, we require that prior to the health
screening you fill out the health questionnaire, the
nutrition/exercise forms, the lifestyle motivation form
and sign the consent form that you can access online and
download at our website www.livingheartfoundation.org
by clicking on the Health Record Login button and
entering the username and password "lhfscreening."
For computer processing reasons, do not change this
prescreening form access code. You also have the option
of calling us and requesting that we send you the forms
by email or post office delivery if you have technical
difficulties. Whichever means you decide to
use to acquire the screening forms, please bring your
completed forms with you to the health screening where a
member of the LHF staff will review them and answer any
questions you may have. Although your individual health
results are confidential, we ask you to permit us to
analyze your de-identified data as part of a group
analysis of many professional football players. In this
fashion, the LHF can learn about health trends, which
could potentially help all football players, young and
old. If you have questions and want to reach the LHF
office prior to the screening, please call and a member
of the LHF team will promptly return your call if we are
not available at that time. The health screening process
involves a morning finger-stick blood test, which takes
about three minutes per person. You should fast for 12
hours prior to this test for the most reliable results.
The blood testing will start at 8am. The entire
screening process, which is described in the program
description, is very complete including specific tests,
health education and counseling. Plan to spend two to
three hours for maximal benefit
As a former player, I want our CV
Health Screening Program to work for you. You can be
sure that the LHF staff and its regional hospital
partners will
give you our best effort. Our main concern is your good
health over the long-term. You will find enclosed a
description of our CV Health Screening Program, which
will provide you with more detailed information. On your
health-screening day, you will receive your personalized
LHF health identification card that you can use to help
make your health information accessible by website to
doctors and nurses 24/7 especially under emergency
conditions or while you are traveling. You will receive
some immediate health results at the screening and these
will be discussed with your by a LHF associated
physician. The remainder of your screening tests will be
interpreted and sent back to you within six weeks.
You are encouraged to communicate with the LHF by
website or telephone for health guidance after the
screening. The LHF features special programs in
nutrition, appropriate exercising methods in the face of
arthritic bodies, sleep apnea, and the metabolic
syndrome.
In order to properly plan
for the 2-day screening event, we ask that you choose a
day to attend and complete an identifying information
form, which can be accessed by clicking the contact LHF
link below. We plan on
screening 30 current or retired players per day.
After we
receive your email registration inquiry or phone
call, we will contact you with
details of your health screening day. The LHF feels that
it is especially important to be proactive in addressing
our own health issues. By this action, we also send an
important message to the youth of America who have
looked up to us as examples – care about your health,
undergo health check-ups and learn how to protect your
future quality of life.
