The HeartLife personal health register is a means to record, track and access an individual's medical status.   .   The Living Heart Foundation will use this new program for it's public security and "Health for Life" initiatives.

Your health information is personal.  You can decide who should be permitted to view your health condition.  If  you are in a school environment, you can choose to share your information with certain people associated with your institution including coaches, parents, athletic trainers, school physicians, and family doctors.  If you are employed, you can share your medical data with your company physician, if there is one, a workplace nurse, or your private physician.  The choice is yours and your medical information is confidential.  Only you can approve of others having access to your health status.

Once your health information is entered into your HeartLife health record, you can access such data from any location where a functioning computer is present.  In addition, you can build on your baseline health history by inserting new information as it is obtained days, months, or yours later.  In the process, you can create the ability to track your own health progress as  you advance through your school y ears and later become a working adult.  If you change your address or take a trip, your medical record is always available within minutes with your health history registry.  In fact, in an emergency, other health care providers who need to know your health data can have this information without valuable time lost.

In our "Health for Life" program, the individual member confers with the athletic trainer or other primary health care provide on a regular basis.  During these sessions, you and the trainer utilize the computer to access your health record and update  your important medical facts.  Questions that remain outstanding are referred by email to our medical faculty and responses are returned within a few days.  In addition, health information obtained from outside sources like laboratories, hospitals, or personal physician offices can also be entered into your record by you,  your own doctor, or other individuals authorized by you.

The HeartLife health record is yours to keep for life.  Whenever and wherever you desire this special data, it is available to you.  As the leader of your own health status, you determine what doctor(s) or other important individuals are permitted to view your health record.
